Insurance Information

Tests done at Michiana Regional Sleep Disorders Center are outpatient diagnostic procedures. We suggest that you check with your insurance carrier for coverage on your individual policy. All outstanding balances are due by the patient within 90 days of the test date. Your insurance carrier may ask you for diagnostic and procedure codes, which are as follows:

Diagnostic:
Obstructive Sleep Apnea 327.23
Narcolepsy 347.0

Procedure:
Overnight Polysomnogram Study (NPSG) 95810
Multiple Sleep Latency Test (MSLT) 95805
NPSG with CPAP Titration 95811

We also suggest that you question your insurance company about DME (Durable Medical Equipment) coverage in the event that you require treatment with NCPAP (Nasal Continuous Positive Airway Pressure), Bilevel Pressures or nocturnal oxygen. Your insurance may have a preferred provider, please let us know if you are instructed to use a certain durable medical equipment company. The sleep center provides equipment for those patients whose insurance company participates with our center for Positive Airway Pressure devices and supplies. Please call the sleep center if you have questions.
